I like this idea. I would also suggest including a 'template' card that looks like the quilt, with numbers where each square goes, that tells the winner which game the square is from and who made each quilt square (user name or real name).
Personally I REALLY like this idea, and it should be easier to do than send a card all around the world. I think it would be cool to also put roughly where people are from. For example my entry would be "Dr. Mario - crabbyman, PA, USA" or something like that. It would make it clear that its a world-wide community of people that worked on the quilt and that support the charity. I don't think it has to be super detailed, but I think it would be a pretty cool personal touch to the map card.

You guys really want to make more work for me don't you? Seriously though, it sounds like a good idea. Once I have the quilt laid out the way I am going to do it, I can start to gather everyones informtion. I have print shop so it shouldn't be a problem to work something up. I just need to make sure I get everyones information in order to make sure it doesn't have any holes.

Don't send me anything now, trust me, I will misplace it because I am worrying about the quilt itself. I will get grid up when I know the placements and everyone can fill them in.

When we decide, make sure we get it posted on the Sprite Stitch Facebook page. Then, we can all link to it and do status updates about it-- thus getting publicity out for the quilt AND for Sprite Stitch itself!

Ok, so the consensus looks like we should send it to the dinner. Hopefully we will have more then one next year if we do it again so we can auction one off ourselves.

I will contact Child's Play and let them know and ask them to send me the forms.

I don't have a Twitter account, but my husband is @sterling_m and I think that GamesRadar also has an official Twitter account where they posted a link to the article. As for Facebook, I've just linked the article itself.
